The IATA Shipper's Declaration is used when shipping hazardous materials and dangerous goods by air. It contains information such as the shipper's name and address, the cosignee's name and address, the departure and destination airports, dangerous goods identification, proper shipping name, class or division, UN or ID number, subsidiary risk, quantity, type of packaging, and additional handling information.

These air declaration forms meet the requirement of IATA, section 8 & ICAO regulations. All shipments of dangerous goods require the use of an air declaration form.
